LEAGUE MEDICAL/LIABILITY:

• I declare my child is physically and mentally sound and suffering from no condition, impairment, disease, infirmity, or other illness that would prevent participation in Savannah Coastal Swim League (hereinafter SCSL).
• I acknowledge I have been informed of the need for a physician’s approval for my child’s participation in an exercise/fitness activity. I recognize it is my sole responsibility to obtain an examination by a physician prior to involvement in swim team activities. I acknowledge
• I have either had a physical examination and been given my child physician’s permission to participate, or if I have chosen not to obtain a physician’s permission prior to beginning swim team activities, I acknowledge I am doing so at my own risk.
• I certify that I am the parent or legal guardian for my child. I hereby give my permission for any supervisor, coach or other league administrator associated with the SCSL to seek and give appropriate medical attention for our child in the event of accident, injury, illness. I will be responsible for any and all costs associated with any necessary medical attention and/or treatment.
• I hereby waive, release and forever discharge SCSL and associated coaches or other league administration from all rights and claims for damages, injury, loss to person or property which may be sustained or occur during participation SCSL activities, whether or not damages or loss is due to negligence.
• By registering my child to participate with any of the member teams of SCSL, I agree to participate (or allow my child and family members to participate) in the SCSL, and hereby release SCSL, its directors, officers, agents, coaches, and employees from liability for any injury that might occur to myself (or to my child and family members) while participating in the SCSL programs, including travel to and from practices, swim meets or other scheduled team/league activities.
• I agree to indemnify and hold harmless the above mentioned organizations and/or individuals, their agents and/or employees, against any and all liability for personal injury, including injuries resulting in death to me, my child and/or other family members, or damage to my property, the property to my child and/or other family members, or both, while I (or my child or family members) participating in the SCSL programs.

MEDIA/PHOTO RELEASE:

• I agree for my athlete to be photographed for promotional purposes for SCSL. I understand that the athlete’s image and likeness may be used for print, electronic or web-based communications. I further understand that I will not be compensated monetarily now or in the future for my image or for this activity.
• I agree to grant to SCSL, its licensees, and producers of its educational and promotional materials and their successors and assigns, the right to use, publish, and copyright my picture, voice and/or moving image for advertising and promotion materials.
• I agree that I am the Parent or Guardian of athlete, a minor, and that I have read and approved the above media/photo release.
• I agree to release SCSL and all of its officers, employees, and agents from any liability claims and costs of whatever kind that occur in connection with my actions or the actions of the above individual.

• By registering my child with the Mayfair Marlins Swim Team, LLC, I agree to participate (or allow my child and family members to participate) in the Mayfair Marlins Swim Team, LLC, and hereby release Mayfair Marlins Swim Team, LLC, its directors, officers, agents, coaches, and employees from liability for any injury that might occur to myself (or to my child and family members) while participating in the Mayfair Marlins Swim Team, LLC programs, including travel to and from training sessions, swim meets or other scheduled team activities.
• I agree to indemnify and hold harmless the above mentioned organizations and/or individuals, their agents and/or employees, against any and all liability for personal injury, including injuries resulting in death to me, my child, and/or other family members, or damage to my property, the property to my child and/or other family members, or both, while I (or my child or family members) participating in the Mayfair Marlins Swim Team, LLC programs.
• Releaser expressly agrees that this release, waiver and indemnity agreement is intended to be as broad and inclusive as permitted by the laws of the State of Georgia, and that the balance shall, notwithstanding, continue in full force and effect.

The undersigned athlete participating with/for the Mayfair Marlins Swim Team, LLC agrees to abide by the guidelines outlined below in addition to those established by team leadership.

• Swim for the fun of it, not just to please your parents or coach.
• Make every team practice, meet participation and activity an opportunity to learn.
• Represent MMST with excellence, respect, team spirit and politeness.
• Follow the directions of the coaching staff, respect official instructions and designated chaperones. Disrespect or failure to obey instructions will not be tolerated from any athlete.
• Practice good sportsmanship at all times to the team members, coaches, competitors, officials, parents and for all facilities and properties used during practices, competitions and team activities.
• Be punctual and arrive on time for all practices, meets and team events. Pool time is very valuable.
• Be an active participant in all team practices, meets, and other team activities.
• Swimmers must notify a coach, in advance, if they are planning to leave practice or swim meets early.
• Wear MMST team caps and appropriate (team) swim suits at all swim meets.
• Disruption of practice by an athlete will be grounds for removal.
• Pay attention and follow all of the coach’s instructions completely and exactly. Ifclarifications areneeded, ask questions politely.
• Use appropriate language. Use of profane or abusive language or obscene gestures will not be tolerated.
• Dishonestly, theft, and/or vandalism will not be tolerated.
• Indiscreet or destructive behavior will not be tolerated.
• Swimmers are expected to respect each other. Fighting, intentional touching or striking another athlete will subject the swimmer to the most severe discipline.

Swimmers are expected to follow the spirit of the rules as well as the specific rules. The coach must adapt this philosophy to an infinite number of situations. Swimmers are asked to respect the coach’s directions and give their full cooperation. Cooperation with teammates and coaches will produce a productive practice environment and competitive environment for all.

The following penalties for violating the MMTS's Code of Conduct include, but are not limited to, the following penalties:

The swimmer will be given a verbal warning and/or will be pulled out of practice in addition to a verbal warning, and the coach will contact the parent.

If the swimmer's disciplinary problem continues, the swimmer and parent will meet with the coach to discuss the problem further.

If the swimmer continues his/her bad behavior, he/she will be suspended for the season with no refund (there will be NO prorated fee).

• I agree to release MMST and all of its officers, employees, and agents from any liability claims and costs of whatever kind that occur in connection with my actions or the actions of the above individual(s).

If you are not a member of the Mayfair Pool for the 2024 season, there is a $50 "Splash fee" per family for non-members. If you do not join the Mayfair pool and don't check box now - your family will be charged the additional $50 splash fee. DO NOT CHECK BOX BELOW "we understand..." if you plan to join the pool. 

• I agree that I am personally responsible for my safety and actions while using the Mayfair Pool. I agree to comply with all Mayfair Pool policies and rules, including but not limited to all policies, guidelines, signage, and instructions. I understand the pool risks as posted.
• Because the Mayfair Pool is open for use by other individuals, I recognize that I am at higher risk of contracting COVID-19. With full awareness and appreciation of the risks involved, I, for myself and on behalf of my family, spouse, estate, heirs, executors, administrators, assigns, and personal representatives, hereby forever release, waive, discharge, and covenant not to sue the Mayfair Pool, its board members, officers, agents, servants, independent contractors, affiliates, employees, successors, and assigns (collectively the "released parties") from all liability, claims, demands, actions, and causes of action whatsoever, directly or indirectly arising out of or related to any loss, damage, or injury, including death, that may be sustained by me related to COVID-19 whether caused by the negligence of the released parties, any third-party using the Mayfair Pool, or otherwise, while participating in any activity while in, on, or around the Mayfair pool and/or while using any Mayfair facilities, tools, equipment, materials.

City of Savannah, County of Chatham, State of Georgia, herein referred to as the releaser.

• In consideration of being permitted to participate in swimming, sliding, and diving activities conducted by MAYFAIR PARK, INC., Releaser, for himself, his spouse, legal representatives, heirs, guests, and assigns, hereby releases, waives, and discharges MAYFAIR PARK, INC., its officers, directors, agents, and employees referred to as releases, from all liability to the releaser, his spouse, legal representatives, heirs and assigns, for any and all loss or damage, and any claims or damages resulting therefrom, on account of injury to releaser's person or property, even injury resulting in death of the releaser, whether caused by the negligence of releases or otherwise, while the releaser is participating in swimming, sliding or diving activities. 
• Releaser expressly agrees that this release, waiver and indemnity agreement is intended to be as broad and inclusive as permitted by the laws of the State of Georgia, and that the balance shall, notwithstanding, continue in full force and effect.

We understand that it takes a village and everyone must pitch in for summer swim league. In order to stay on the team next year, we must volunteer a minimum of two meets - whether home or away. These meets could be dual (6), Rising Star or the City/Championship meet.
